Grandville Select Boys & Girls will be held WEDNESDAY, JUNE 25th at Century Park (5710 Kenowa SW, Grandville, 49418)  from 5:30-7:30pm

 

If you cannot make your specified registration time, please contac the FIRE office to make alternate arrangements.

Registration Paperwork

Select / Academy Necessary Items

If you are registering your son / daughter for either a SELECT or an ACADEMY team, please make sure you bring the following paperwork to your registration date:

 ***Per the Grand Valley Soccer Association fee structure, any fall player registrations received after 8/02/08 will be subject to a $10.00 late fee.  Any fall player registrations received after 8/20/08 will be subject to a $20.00 late fee.  All late fees are made payable to GVSA.***

Premier / MRL Necessary Items

If you are registering your son / daughter for either a PREMIER or a MRL team, please make sure you bring the following paperwork to your registration date:

  • MSPSP Registration Form (2 ply form found at registration)
  • MSYSA Medical Release Form
  • MSYSA Consent to Travel Form
  • Illinois Medical Release Form 
  • Payment Contract  (2 ply form found at registration)
  • Copy of Player’s Legal Birth Certificate
  • 1" x 1" Photo of Player (please write the player’s name and age division on the back)
  • Volunteer Information Form

***Per Michigan State Premier Soccer Program fee structure, any fall player registrations received after 7/20/08 will be subject to a $20.00 late fee.  Any fall player registrations received after 7/27/08 will be subject to a $40.00 late fee.  All late fees are made payable to MSPSP.***
