Insurance Waiver

According to state sanctioning, only USYSA registered teams will be covered by our tournament insurance.  All teams registered through alternate organizations such as US Club Soccer, AYSO, etc....will need to provide a waiver for each player before they will be allowed to participate in the Soccerfest Tourney.

Roster Form

A Soccerfest Roster Form listing all players and guest players must be filled out and completed by a team official and presented at the time of team registration.  No team will be allowed to register without a completed roster form.

Medical Release

A medical release form allowing for another supervising adult to give consent for treatment of the injured player, must be provided by in order for a player to participate in the Soccerfest tournament.  All medical release forms must be individually noterized and presented at team registration.

Guest Players

Carded - The "carded" guest player is a guest player that is rostered with a USYSA team.  They would already  have a passcard issued to them from their "home" league.  This player needs to fill out the following form:  Carded Guest Player - no passcard needed.

Uncarded - The "uncarded" guest player is a player that is NOT registered with a USYSA team - they may be with a team that is registered with US Club, AYSO, etc...or no team at all.  This player needs to follow the instructions given on the MSYSA website:  Uncarded Player - needs passcard. 

The MSYSA website has two different processes for these two types of players.  For the "carded" player, it is a small fee, and guest player paperwork that lets the player play for another USYSA team.  For the "uncarded" player, there is a slightly higher fee, and they need to prove their age, and will be issued a USYSA player card (the card is usually issued by the league after notification from MSYSA.)

Risk Management
The State and National soccer organizations require that all adults in contact with youth soccer obtain a Risk Management review.  To help protect and maintain high standards, the Lakeshore Soccefest committee reviews all coaches and team managers against the state and national lists of adults that are banned from participating in USYSA soccer.  A current risk management card is required by all team officials in order for them to participate and coach in the Soccerfest Tournament.
